ABSTRACT

INTRODUCTIONAcute fatty liver of pregnancy (AFLP) frequently causes liver failure in pregnant women. A better understanding of the clinical characteristics, management, outcomes and risk factors of AFLP is required, given its relatively high mortality rate. We aimed to describe the characteristics of AFLP, and further assess its outcomes and potential risk factors from the perspectives of the mother and fetus.

METHODSThis was a retrospective cohort study of 133 patients with AFLP hospitalised at four tertiary hospitals in China between January 2009 and April 2014.

RESULTSAmong 133 patients, AFLP was diagnosed in the postpartum period for 13 (9.8%) patients. Potential factors influencing adverse maternal outcome were male fetus (p = 0.04), postpartum diagnosis of AFLP (p < 0.01), intrauterine fetal death (p = 0.04), disseminated intravascular coagulation (p < 0.01), prothrombin time (p < 0.01) and activated partial thromboplastin time (p = 0.04). The frequency of fetal distress (p = 0.03) and activated partial thromboplastin time (p < 0.05) were significantly higher in pregnancies with dead fetuses than in those where the fetuses survived. Independent risk factors for perinatal maternal mortality were history of legal termination of pregnancy (odds ratio [OR] 1.958, 95% confidence interval [CI] 1.133-3.385), total bilirubin (OR 1.009, 95% CI 1.003-1.014) and serum creatinine (OR 1.010, 95% CI 1.003-1.017).

CONCLUSIONCompared with total bilirubin and serum creatinine, history of legal termination of pregnancy appeared to be a greater risk factor for maternal mortality among patients with AFLP.

ABSTRACT

OBJECTIVETo evaluate the clinical effect of traditional Chinese spinal orthopedic manipulation (TCSOM) in treating patients with functional abdominal pain syndrome (FAPS) in comparison with Pinaverium Bromide (Dicetel, PBD), and to assess a possible cause for FAPS.

METHODSEighty patients with FAPS were randomly and equally assigned to the TCSOM group and PBD group according to the random number table. All patients in the TCSOM group were treated with a maximum of 5 times of spinal manipulations. Patients in the PBD group were instructed to take 50 mg 3 times a day, consistently for 2 weeks. The symptoms of pre- and post-treatment were assessed on a visual analog scale (VAS) pain score. A symptom improvement rating (SIR) was implemented to evaluate the effects of the treatments.

RESULTSThe symptoms of 27 cases of the TCSOM group were relieved soon after the first TCSOM treatment and 9 cases were significantly improved. The VAS pain scores in the TCSOM group were significantly lower than those in the PBD group after 2 weeks treatment. According to the SIR based on VAS, the TCSOM group included 30 cases with excellent results, 7 cases with good, and 3 cases with poor. Adverse events to the treatment were not reported. Based on VAS, the PBD group reported 8 cases with excellent results, 10 cases with good and 22 cases with poor. There was a significant difference between the two groups (P<0.01).

CONCLUSIONSThe displacement of intervertebral discs and/or vertebra in the thoracic or lumbar region seems to be a contributing factor in the symptoms of FAPS. TCSOM is an effective treatment for FAPS.

ABSTRACT

Objective: The non-convulsive status epilepticus (NCSE) is easy misdiagnosed due to its complex etiology, clinical manifestations. It is more difficult to diagnose and treat for elderly patients with multiple basic diseases. The purpose of this study is to investigate the clinical characteristics of elderly patients with NCSE, therapy, diagnosis, and the value of long-range video electroencephalography (LV-EEG). Methods: The clinical and electroencephalography (EEG) data of the five elderly patients with NCSE in my hospital were analyzed retrospectively. Results: Five patients showed episodes of abnormal behavior during seizure (5/5). There were 3 patients with previous history of epilepsy after cerebral infarction (3/5), 1 patient diagnosed intracranial infection because of NCSE episodes to consultation (1/5), and 1 case with previous history of epilepsy (1/5). The V-EEG of the 5 patients showed persistent epileptic form discharges during seizure. Intravenous diazepam had good effect to the 5 patients during seizure. Seizure of the 5 cases was controlled acceptably with different antiepileptic drugs according to different epilepsy forms. Conclusions: The abnormal mind and behavior is still main performance in elderly patients with NCSE. The LV-EEG provides reliable foundation for diagnosis of the disease. The rational use of antiepileptic drugs is effective. Intravenous diazepam slow for elderly patients with NCSE is applied as well.

ABSTRACT

BACKGROUND:How to obtain a sufficient number of cells is one of the key issues in the celltransplantation therapy, and studies have shown that stem cellproliferation can be promoted by reasonable stimulus. OBJECTIVE:To investigate reduced glutathione effects on biological characteristics of human umbilical cord blood mesenchymal stem cells. METHODS:The cells were divided into two groups:the control group consisted of the normal human umbilical cord blood mesenchymal stem cells, and in the experimental group, human umbilical cord blood mesenchymal stem cells were treated with 0.15 g/L reduced glutathione. RESULTS AND CONCLUSION:At days 5, 7, 9, cells treated with 0.15 g/L reduced glutathione showed higher absorbance values than those in the control group (P<0.05). Flow cytometry showed reduced glutathione had no effects on CD29, CD44, CD45, CD105 expression. Real-time PCR results showed reduced glutathione was capable of promoting extracellular signal-regulated kinase mRNA expression (P<0.05). Findings from this study showed that 0.15 g/L reduced glutathione can promote the proliferation of human umbilical cord blood mesenchymal stem cells.

OBJECTIVETo study the relationship between nucleotide excision repair gene ERCC1 and resistance to cisplatin in ovarian cancer.

METHODSThe expression of gene ERCC1 in 58 ovarian cancer tissues and 4 cell lines were examined and its relationship with resistance to cisplatin were analyzed, the changes of sensitivity to cisplatin were observed after interference of ERCC1 gene with small interfering RNA (siRNA) in ovarian cancer cell lines.

RESULTSIn 58 ovarian cancer tissues, the positive rate of ERCC1 protein in chemoresistant cases (57.89%) was higher than that in chemo-sensitive cases (28.21%, P = 0.029). The mRNA levels of ERCC1 gene in ovarian cancer cell lines ES-2, SKOV3, COC1, COC1/DDP were related to cisplatin IC50 values (r = 0.932, P <0.05). The sensitivity of cell lines ES-2, SKOV3, COC1/DDP cells to cisplatin was increased by 53.88, 5.07, and 3.75 times, respectively, after RNA interfering ERCC1 gene.

CONCLUSIONERCC1 gene is associated with the resistance to cisplatin and the sensitivity to cisplatin can be enhanced by RNA interfering ERCC1 in ovarian cancer.
